SUBCONTRACTOR AND AUTHORIZED FULFILLMENT PARTNER AUTHORIZATION

Contractor to fill out and submit all information below for approval of Subcontractors and Authorized Fulfillment Partners by Enterprise Services. Upon

verification, Enterprise Services will respond to Contractor in writing to confirm approval, deny approval, or request additional information.

All fields are required prior to authorization by Enterprise services. If additional space is needed, table may be copied into an excel document for submission,

provided all fields are included and file name is copied.

• Business Name: Enter business’ legal name. DBAs can be provided additionally but are not required.

• Contact Name: Use best contact name for direct from Purchaser communications.

• Contact Email and/or Phone: Provide either or both but must include one.

• UBI#: Unified Business Identifier (UBI) is a 9-digit number and needed for verifying their DOR and SOS presences.

• FEIN#: Federal employer identification number is a 9-digit number and needed for verifying their WEBS presence.

• Statewide Payee #: To receive payment from customers, business must be registered with the Office of Financial Management’s Statewide

Vendor/Payee Services.

o Optional for Subcontractors. Required for Authorized Fulfillment Partners

• WEBS: Businesses are required to be registered in Washington’s Electronic Business Solution for Vendors.

• DOR/SOS: Businesses are required to be registered and active with both Washington State Department of Revenue and Washington Secretary of State.

• Small/OMWBE/Veteran: Enter Yes for any WA certifications, as defined in PA section 6(c) or 7(e). Enter No for none or not applicable. This field is

optional for both Subcontractors and Authorized Fulfillment Partners.
